About Me

Tarren Evans grew up in Wichita, Kansas playing sports and volunteering in her local church community. She went on to receive minors in sociology and psychology at Wichita State University before turning toward the medical and business side of population health. She received her Bachelor’s degree in Business Administration from the University of Arizona Global Campus and a Professional Management certificate from the University of Notre Dame Mendoza College of Business. Tarren has over 10 years of experience in various roles in hospital systems operations and leadership. She and her family reside in Grove, Oklahoma on Grand Lake O’ the Cherokees. She is a board member of Delaware County Children’s Special Advocacy Network, an organization that brings support and hope to thousands of families facing abuse. She enjoys cheering on her kids in sports and band competitions, volunteering in the community, and staying active by paddle boarding, golfing and running.

My Why

Growing up, my parents were always coaching, volunteering, and helping others. It was instilled in me to be a servant leader. My healthcare career began as a leader of a medical scribe program in the emergency department. The opportunity had provided me with a small glimpse of how the healthcare ecosystem works and lended me insight into multi-faceted barriers to care in which many face. I was inspired by the people that impacted me throughout my journey working in the emergency room and I wanted to do more for patients. In 2018, Vituity’s Innovation and Transformation department offered Innovation Grants to local provider groups to encourage them to solve an issue with a creative new idea. The scribe program applied for a grant to understand, via a patient care navigator, why people were returning to the ER for basic care. The program’s outcome had proved a more than 50% reduction in high utilizer emergency room visits, saving millions of dollars in healthcare costs, just by listening to people, understanding their deep rooted issues with accessing healthcare, and providing guidance to steer them toward a healthier journey. Anywhere from post-acute to inpatient, and spanning specialty populations, patient care navigation is a necessity in closing gaps in healthcare. To further my intense passion for guiding people through the complex web of healthcare, I found myself in need of guidance and support. A close family member had been struggling with substance use and was willing to accept treatment. I helped them navigate the logistics: Where to go, costs, insurance complexities, the admission process. It was grueling, to say the least. There I was, having many years of what I thought was a deep knowledge of navigating the system, yet spending days upon days making call after call, asking question after question, and researching intensely. I realized just how impactful our navigators are in people’s lives. My loved one fortunately had my help in digging through the trenches of the system. Many others don’t have that kind of support. My loved one told me, “If you weren’t helping me through this, I would have given up”. My mission is to ensure no one else feels so lost and helpless in finding help that it’s easier to just give up. I believe that patient care navigation can give that hope back to the community, and Rely Health is here to make that possible.
